Creating Google Forms

You can create a form from the Docs list or from any spreadsheet. Go to docs.google.com and log-in.

Creating a form from your Docs list:

Click Create new > Form.
In the form template that opens, you can add any questions and options you’d like.
Click Email this form once you have finished adding your questions.
Add the email addresses of the people to whom you want to send this form.
Click Send.

Creating a form from a spreadsheet:

Click the Form drop-down menu and select Create a form.
In the form template that opens, you can add any questions and options you’d like.
Click Email this form once you have finished adding your questions.
Add the email addresses of the people to whom you want to send this form.
Click Send.

Sample Feedback Form

After people have filled out your form, you can view analytics for your form data by clicking Show Analysis in the edit form view. All of your form responses are stored in a Google Spreadsheet, which can be easily exported to .XLS/.CSV for making custom graphics in Excel.
